  Looks like the image - the little teddy bear is clipped onto the dress (and therefore optional), the ribbons on the neck/chest have to added by yourself which does let you add your own ribbons if you wish (or to just go without the ribbons too). the waist is elastic, so I would recommend checking size by comparing bust instead of waist size (I personally did this and the dress fits great)
